After a DOT drug or alcohol violation, the employer must give the employee a list of SAPs that are readily available to the employee and acceptable to the employer, with names, addresses and phone numbers, at no charge to the employee under 49 CFR 40.287. This tool builds a list using the driver's state and meeting-format filters and includes listed SAPs with a direct phone number. Timing and language filters are for directory discovery and are not used to build the employer PDF.
The list is dated and shows the filters used. Before giving it to the employee, confirm that each SAP is currently qualified, readily available and acceptable to your company. SAPs maintain their own profile information. Paid membership affects directory features and direct-contact visibility; it does not establish DOT qualification or approval.

You are removed from safety-sensitive work
Your employerAfter a DOT drug or alcohol violation, you cannot perform DOT safety-sensitive functions. For FMCSA-regulated CDL drivers, the violation is reported to the Clearinghouse. You remain prohibited from safety-sensitive functions until the required SAP process is completed and a negative return-to-duty result is reported to the Clearinghouse.

Choose and designate a SAP
YouYour employer must give you a list of SAPs that are readily available and acceptable to the employer. You can choose from that list or find a SAP through your own research. Contact the SAP first, then designate that SAP in the FMCSA Clearinghouse.

Initial evaluation
Your SAPThe SAP evaluates you in person or, at the SAP's discretion, by live video where their professional credential permits it. A remote evaluation must meet DOT audio, video, security and geographic requirements. The SAP recommends the education and/or treatment you must complete.

Complete the education or treatment
YouComplete the SAP's recommendations. The SAP then conducts a follow-up evaluation and determines whether you have successfully complied.

Return-to-duty test
Employer or applicable C/TPAAfter the SAP determines that you have successfully complied, the responsible employer arranges the return-to-duty test if it intends to return you to safety-sensitive work. A return-to-duty drug test is directly observed and must be negative. A return-to-duty alcohol result must be below 0.02. For FMCSA drivers, the responsible employer or authorized C/TPA reports the qualifying result to the Clearinghouse.

Follow-up testing
Your employerIf you return to safety-sensitive work, the SAP's plan must include at least six unannounced follow-up tests during your first 12 months of safety-sensitive duty. The SAP may require testing during up to another 48 months of safety-sensitive duty. The plan follows you to subsequent employers and through breaks in service.
No current or prospective employer? FMCSA says an eligible driver may register in the Clearinghouse as an owner-operator and designate a C/TPA for the limited purpose of completing the return-to-duty process. This does not guarantee employment or restore a CDL by itself.
Once a SAP has evaluated you, you must not seek a second SAP evaluation simply to obtain a different recommendation.
For employers after a DOT violation
- Remove the driver from DOT safety-sensitive functions when required by the applicable DOT rules.
- Give each employee with a DOT drug or alcohol violation, including an applicant or new employee, a list of SAPs that are readily available and acceptable to you, with names, addresses and phone numbers, at no charge under 49 CFR 40.287. This duty is not avoided simply because you terminate the employee or decide not to hire the applicant. If you use a directory, confirm the SAP's qualifications and availability before giving the list to the employee.
- If you decide to return the driver to safety-sensitive work, ensure the required return-to-duty test is completed and carry out the SAP's follow-up testing plan. Random testing does not replace required follow-up testing.

Questions drivers ask about the SAP process
Can I choose my own SAP?
Yes. Your employer must give you a list of SAPs that are readily available and acceptable to the employer, but FMCSA drivers may choose a SAP from that list or through their own research. Contact the SAP first, then designate the SAP in the Clearinghouse. Once a SAP has evaluated you, you cannot seek a second SAP simply to obtain a different recommendation.
Can the SAP evaluation be done online?
Yes. At the SAP's discretion, a DOT SAP evaluation can be conducted remotely if 49 CFR 40.291 is met and the SAP's professional credential permits it. The evaluation must use real-time audio and video of sufficient quality and security, and the SAP must stay within the geographic limits of their credential.
Does the SAP process end when I pass the return-to-duty test?
No. A qualifying return-to-duty result can remove the FMCSA safety-sensitive-work prohibition once it is properly reported, but the SAP's follow-up testing plan still applies. The plan must include at least six unannounced tests during the first 12 months of safety-sensitive duty and may require testing during up to another 48 months.
Do I need a SAP if I left the job after the violation?
Yes, if you want to complete the DOT return-to-duty process and resume DOT safety-sensitive work. Leaving the employer where the violation occurred does not remove the return-to-duty requirements.
What does the SAP report to the Clearinghouse?
For an FMCSA-regulated driver, the designated SAP reports the date of the initial SAP assessment and the date the SAP determines the driver is eligible for return-to-duty testing. The SAP does not enter treatment details or the follow-up testing plan into the Clearinghouse. The responsible employer or authorized C/TPA reports the qualifying return-to-duty result and, later, completion of the follow-up testing plan.
Can a SAP return my CDL to active status?
No. The SAP determines whether you have successfully complied with the required education or treatment and are eligible for return-to-duty testing. The SAP does not reinstate a CDL or guarantee employment. For FMCSA drivers, the qualifying return-to-duty result must be reported to the Clearinghouse by the responsible employer or authorized C/TPA before the driver's Clearinghouse status can change from prohibited to not prohibited.
