Initial and follow-up evaluations
The initial evaluation establishes the SAP’s education or treatment recommendations. A follow-up evaluation occurs after the employee has carried out the recommended steps and allows the SAP to assess successful compliance. Buying a course or completing an appointment alone does not finish the DOT return-to-duty process.
Checking qualifications for Arizona
When arranging an evaluation in Arizona, distinguish the employee’s actual location from the employer’s address. Arizona’s Board of Behavioral Health Examiners provides a directory and verification service for the professions it regulates. If the SAP relies on an Arizona license in a profession regulated by the Board of Behavioral Health Examiners, use that board’s verification tool; otherwise use the relevant licensing board or credentialing body. Preparing for an evaluation
