FMCSA may conduct a safety audit at the carrier’s place of business or offsite through the New Entrant Web System. For an offsite audit, the carrier receives instructions for submitting documents electronically or by mail or fax. FMCSA advises carriers to keep their contact information current because the agency uses the registration record to communicate about the audit.

Under 49 CFR 385.321, a violation of any one of 16 listed regulations causes automatic failure. Most use a single-occurrence standard. Two use a 51-percent threshold in the records examined: failure to require records of duty status and use of vehicles without the required periodic inspection. Those 16 rules are not the only way to fail.

For an audit involving a Chicago operation, use the official notice to confirm the contact, submission route and requested period. The reviewer may be FMCSA or its state partner. A consultant’s Illinois coverage does not establish authority to conduct the official audit or promise its outcome.
